Please only fill out if we have discussed a time & date for your session via email or phone call. $100 deposit is required to save the date and reserve your spot. The remaining balance (cash or check) is due on the day of our session. If you decide to cancel, your deposit is non-refundable. A clients request to change the session date due to illness or weather can be rescheduled upon Jen Amato's availability. You will be given several make up dates to pick from. Reschedules must take place within 1 month of original booking date. If not, deposit is no longer valid. Please be sure to add in your session date and time in the below field. You will be directed to PayPal once you sign your name at the bottom. Once deposit is collected, your session date is officially reserved. (If this is for a newborn session, the shoots are $600. Please pay $100 deposit & remaining $500 is due on the day of the session.) * There is a travel fee for sessions in Baton Rouge, Mandeville and Covington.
